First Visit

What will happen on my first visit?

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist (if you have one).
  • A list of medications you are currently taking (please note that you will be sent a link prior to your appointment to complete your medical history form).
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
    • Please note that we are not in-network with any insurance provider. This allows us to provide the best care possible to our patients. If you would like to use your insurance, we will help by submitting claims and facilitating reimbursement directly to you. 

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to your appointment by filling out the health history form to the best of your ability (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension). Many conditions (i.e. diabetes, heart disease, etc.) have a connection to periodontal health and a comprehensive health review is critical to our assessment and, ultimately, treatment outcomes.

During your first visit with us, we will complete an in-depth medical and dental health history and a thorough examination that includes measurements for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease. Additional x-rays may be taken to allow for assessment of bone levels and tooth conditions.

After the examination, we will review the findings and make recommendations and suggestions which may include oral hygiene instructions (with new instruments/brushes provided to you).
